結節性硬化症に対するargon laserを用いた光凝固療法例

A case of retinal tumors of tuberous sclerosis treated by the laser photocoagulation

　我々は発生部位として比較的めずらしい黄斑輪にかかる網膜腫瘤の出現した結節性硬化症の1例を経験し，argon laserを用いた光凝固療法を行ない，併せて腫瘤の局在について統計的考察を行なって以下の結果を得た。

（1）黄斑輪にかかる網膜腫瘤の症例に対して，argon laserを用いて光凝固療法を行なって良好な結果を得た。

（2）本邦における報告例と自験例の統計的考察により，網膜腫瘤の局在は，乳頭に近いほど多いこと，耳側と鼻側の間には差が認められないこと，黄斑輪にかかる頻度の少ないことから，この腫瘤は網膜血管の解剖学的位置と密接な関係があると思われる。

　我々は網膜腫瘤の位置，視機能に及ぼす影響，悪性度により光凝固療法を行なって十分な効果を期待できると考えるが，結節性硬化症の本態や網膜腫瘤の性質がまだ不明であり，光凝固による腫瘤の散布の可能性も否定できないので，十分な検査・経過観察を行なって，慎重に光凝固療法の適応を決定する必要がある。

A male child with tuberous sclerosis demon-strated retinal parafoveal tumors with disturbed ring reflex bilaterally at the age of 20 months. Both eyes were treated by argon laser photocoagu-lation. Four months after treatment, the tumors became smaller and atrophic.

We evaluated the topological distribution of retinal tumors in tuberous sclerosis in our own 4 cases including the present one (7 eyes) and 9 reported cases (13 eyes) in the literature. They were more frequently located nearer to the optic disc.Temporal and nasal retina were similarly affected.
