Behçet病の眼症単一型について

Monosymptomatic ocular type of Behçet's disease

　Behget病に定型的とされている急性・一過性の再発性虹彩毛様体炎（時には前房蓄膿を呈す），ないし出血・白斑をともなう再発性網膜ぶどう膜炎が存在しながら，発症後5年−18年経過後もなお再発性の粘膜・皮膚・外陰症状などの眼外症状が出現していない9症例について，その臨床経過・螢光眼底所見・臨床検査所見・病理組織像を報告した。これら所見はBehçet病にみられる眼症と酷似していることから「Behçet病の眼症単一型」と仮称した。

　このような「眼症単一型」の場合は，とかく原因不明のぶどう膜炎として，ステロイド剤内服療法が第1選択とされやすいが，Bchget病の眼症状に対してはステロイド剤内服療法は視力予後に悪影響をおよぼすので，「眼症単一型」の治療に際しても，ステロイド剤内服療法をさけるというBehçet病治療の原則を守ることを強調した。

Diagnosis of Behçet's disease is currently made in Japan by the combination of four cardinal symptoms, i.e., mucosal, genital, skin and ocular lesions, according to the recommendation by the Research Committee of Behçet's Disease. Depend-ing on the number of existing, cardinal symptoms, cases are classified into four different types : com-plete (four cardinal symptoms), incomplete (three cardinal symptoms, or ocular lesions accompanying one of the other cardinal symptoms), suspect (two cardinal symptoms) and possible type (one cardi-nal symptom).
