シリコンループ留置術の新法による涙点閉鎖の治療

A Newly-Devised Silicone Intubation of the Occluded Lacrimal Puncta

緒　言

　涙小管閉塞，あるいはこれに鼻涙管閉塞をも合併した症例に対して，開放手術を行なわずにシリコンラバーチューブをループ状に留置する二，三の方法^1〜3）がすでに試みられているが，いずれも技術的に共通した難点をもつている。岩すなわち，この柔軟なチューブを涙道へ誘導するための銀製^1）や鋼製^2）の案内ブジーを下鼻道から，あるいは通常後方へ向かつて進むナイロン線^3）を鼻咽頭から探し出しで，これらを鼻孔からとり出すことは容易ではない。私は特にこの問題を解決するとともに，本法をまず涙点閉鎖に適用して，卓効を得たので報告する。

Silicone rubber tubing can easily be introdu-ced into the nasal cavity through the lacrimal passages with a single loop, by the device (Fig. 1, 3) that facilitates the recovery of the two polyethylene guide tubings from the nose. The procedure was successfully applied to persistent acquired occlusion of both the upper and lower lacrimal puncta in all six cases. The loop with openings was left in place for four weeks to relieve epiphora ; the proper time has not yet been determined but probably could be a little shorter.
