視神経疾患における瞳孔対光反応と視機能

Pupillary reactions and visual functions in optic nerve disease

　瞳孔escape現象は，視路の疾患の一つの症候として良く知られている。特に視神経の疾患においてこの現象は顕著である。一方，視神経疾患の別の症候として昼盲が知られている。この現象はEnochらにより視覚疲労現象として詳細に解析されている。

　我々はこの2現象を約30名の視神経疾患に対して測定した。視覚疲労現象はCFFの自覚的測定を連続的に行い測定した。その結果，瞳孔escapeが量的に大きい患者では視覚疲労も量的に大きいことが判明した。この両現象は，視神経における類似の機能異常であると考えられる。

Pupillary escape phenomenon is a well-known sign in patients with disorders of anterior visual pathways, especially in those with optic nerve dis-eases. Day blindness is also known to be another sign of optic neuritis. This sign was analyzed by Enoch in 1978.

We examined these two phenomena in about 30 patients with optic nerve diseases. Pupillary escape phenomenon was measured by infrared television iriscorder during stimulation by relatively intense light. Visual fatigue phenomenon was also measured as the temporal change of CFF (Critical Flicker-fusion Frequency) by subjective method.
