経強膜argon laser毛様体光凝固の眼圧に及ぼす影響について

Changes of intraocular pressure in rabbit and human eyes after transscleral argon laser cyclophotocoagulation

　有色家兎を用いた動物実験において，経強膜argon laser毛様体光凝固の適量決定について検討した。Argon laser system900（Caherent radi-ation Co．）を用いてspot size 200μ，time 1.0secの条件で，powerを1.0Wと1.5Wの2群に分けて経結膜，経強膜的に毛様体の光凝固を行ない，3カ月間観察した。

　眼圧下降は両群共，光凝固3日後に最大となり，その後眼圧は回復の傾向を示したが，1.0W群では2ヵ月後まで，無処置の他眼眼圧にくらべ有意な眼圧下降を示し，1.5W群では3ヵ月後でも眼圧下降は有意であった。

　実験眼の病理組織学的検索により，毛様体の萎縮，線維化がみられたことから，長期間経過後の眼圧下降は房水産生低下によるものと思われるが，比較的短期間における眼圧下降には他の下降機転も検討しなければならない。

　臨床的応用では人眼においても眼圧下降効果が得られることが判明した。合併症もほとんどなく，症例を選ぶことにより臨床上有用であると考えられる。

Ocular hypotensive effects and morphological changes of the ciliary body were investigated following transscleral cyclophotocoagulation using argon laser in rabbit eyes. Two human eyes with secondary glaucoma were also treated by this means.

Rabbit eyes were treated by cyclophotocoa-gulation with the power output of 1.5W for one group and 1.0W for the other.The spot size of 200/i and time duration of 1 second was used for b oth groups. The intraocular pressure before and aftertreatment was measured by applanation pneuma-tonograph (Alcon).
