教室における脳血管撮影の現況

Cerebral angiography in our clinic

I．緒言

　脳血管撮影術（Cerebral angiography）は1927年Egas Monizの発表以来，その術式及び造影剤に次第に改良が加えられ，特に清水の経皮的穿刺法（1937年）が広く採用せられるに及んで，中枢神経系疾患の診断に欠く事の出来ない補助的手段として広く普及するに至ろうとしている。初期に於て使用された造影剤は，副作用の発現率が高く，安全度の高い優れた造影剤の出現が俟たれていた。近年Urografin （Schering A．G．Berlin）の出現により，重篤・不快な合併症は殆んど認められなくなつて来た様である。

　脳血管撮影は1）脳血管の位置の変化，2）血管自体の形状の変化，3）正常には見られない血管の出現の3つを拠り所とし，頭蓋内の血管性病変（動脈瘤・血管腫・動脈硬化等）及びspace-occupying lesion （脳腫瘍等）を診断し，かかる病変の所在並びに病変め種類をも或程度探索し得る方法であつて，脳外科領域に於ては脳室造影法と共に，診断に欠かす事の出来ない手段の一つである。

In this paper, cerebral angiographic study in our clinic was reported in detail. With use of 76% Urograf in, 61 angiographic investigations were carried out on 45 patients in our clinic, but none of the serious complications resulted. Among these, several interesting cases were demonstrated. The author described the problem of contract media, technique and the dangers of cerebral angiography, making reference to other author's reports.
