二重周辺虹彩切除について

Double peripheral iridectomia

1．はじめに

　Diamox等の眼圧降下剤が発見されてから，緑内障に対する虹彩切除の重要性は比較的減じたかのようにみえる。しかしこれらの薬物は緑内障を根本的に治すものではなく急性縁内障に於てはやはり早期に虹彩切除を行うのが最も良い処置ではないかと思う。

　しかし乍ら普通の虹彩切除法で十分な虹彩切除を行おうとすれば，全虹彩切除となり，瞳孔括約筋が切られるから羞明や視力低下等の嫌うべき後貽症を残すことになる。殊に初期の緑内障の場合にこれを起したならば，医師は決して感謝されるものではない。また単一な周辺虹彩切除では，その効果が弱いようである。そこでこれらの欠点を補う為に，これから述べる二重周辺虹彩切除（第1図）を行つたのである。

By the discoveries of such drugs as Diamox which acts as a depressor on the intraocu-lar high pressure, many kinds of treatments against glaucoma seem to have cost their significance. But these drugs do not cure the disease radically. Usually, iridectomy is performed against acute glaucoma, but only one peripheral iridectomy is too ineffective to regulate the intraocular pressure in many glaucomatous eyes. At this stage the authors performed the double peripheral iridectomy to prevent this shortage of a single peripheral iridectomy which introduced in this report.
