緑内障性視神経乳頭陥凹の成因に関する研究

On the pathogenesis of glaucomatous excavation of the optic disc

I．緒言

　鬱血性緑内障並びに単性緑内障に於て，視神経乳頭に病変を生じることは，周知の事実である。単性緑内障は，急性鬱血性緑内障とは対照的に，軽い自覚症状を起すに過ぎないが，進行すれば高度の視力障害と視神経乳頭の陥凹が特徴的であるので，昔はamaurosis with excavation of discと呼ばれていたと言うことである。

　Duke-Elderは1949年，単性緑内障に於ける乳頭の変化は，視神経の栄養血管の硬化によるものであろうと想像し，眼球の後方の血管が主として侵されるならば，視神経の陥凹を来たしても眼圧上昇を起さないが，眼球の前方の血管（シュレム管の部）が，主として侵されるならば，眼圧上昇を起すもので，眼球の前後部の血管が侵されるならば，単性緑内障の定型的な症状，即ち，眼圧上昇と乳頭陥凹を起すと述べている。

The systolic and diastolic pressures of the brachial artery were lower on the average in simple glaucoma than in congestive glaucoma. The relation was the same above the age of 45. The systolic pressure of the central retinal artery was also lower in simple glau-coma. The severe change of the optic disc was most frequently combined with high in-traocular pressure and low pressure of the brachial artery. 6 cases of simple glaucoma progressed even with normal pressure after operative or medical control.
