Abstract. Purpose:To report a case of temporary traumatic glaucoma with progressive enlargement of optic disc cupping following lowered intraocular pressure. Case:A 16-year-old boy was referred to us after being hit by a flying hard baseball the day before. IOP became normalized 5 days later. Optic disc cupping in the left eye continued to enlarge and the cup-to-disc ratio became 0.8 3 months later followed by arcuate scotoma. Laser speckle flowgraphy and magnetic resonance angiography showed reduced blood flow in the optic nerve head and retrobulbar tissue. Conclusion:This case illustrates that glaucomatous changes in the optic nerve head may further progress after normalized IOP in traumatic glaucoma,possibly due to reduced blood flow in the anterior and retrobulbar optic nerve.
