要約 目的：外傷性緑内障で，短期間で眼圧下降したものの，その後も視神経乳頭陥凹が拡大し，緑内障性視野障害をきたした症例の報告。症例：16歳男性が野球ボールで左眼を受傷し，眼圧40mmHgとなった。治療により眼圧は5日後に正常範囲に下降したが，乳頭陥凹はその後も拡大を続け，3か月後には陥凹/乳頭比は0.8となり，それに伴う緑内障性の視野障害を認めた。レーザースペックルフローグラフィと磁気共鳴血管造影（MRA）で視神経乳頭および球後の血流低下を示す所見がみられ，乳頭形態変化に影響した可能性が考えられた。結論：外傷性緑内障では眼圧下降後も乳頭陥凹拡大が進行することがあり，これには組織の血流低下が関係している可能性がある。
Abstract. Purpose：To report a case of temporary traumatic glaucoma with progressive enlargement of optic disc cupping following lowered intraocular pressure. Case：A 16-year-old boy was referred to us after being hit by a flying hard baseball the day before. IOP became normalized 5 days later. Optic disc cupping in the left eye continued to enlarge and the cup-to-disc ratio became 0.8 3 months later followed by arcuate scotoma. Laser speckle flowgraphy and magnetic resonance angiography showed reduced blood flow in the optic nerve head and retrobulbar tissue. Conclusion：This case illustrates that glaucomatous changes in the optic nerve head may further progress after normalized IOP in traumatic glaucoma,possibly due to reduced blood flow in the anterior and retrobulbar optic nerve.
