糖尿病性網膜症の治療について

Studies on the treatment of diabetic retinopathy

緒言

　糖尿病性網膜症の発生頻度は，著者等の統計^1）からみても，又Waite及びBeetham （1935），Kornerup （1957）等の報告からみても，原病の罹患年数との間に平行関係が認められる。又An-dreani （1956）は，網膜症の重症度と罹患年数との間にも，或程度の関連があると述べている。斯くして，一，二の異なつた見解もあるが，網膜症は原病の罹患年数と共に逐次発生且つ進展するものと考えられるのである。

　著者等は最近比較的多数の糖尿病患者を観察する機会を得て，その眼底所見に就て分析的観察を試みた（臨床眼科12巻1673頁1958,13巻765頁1959）。黙るにこれらの症例の中で網膜症を認めた患者の多くは，内科的にかなり厳重な治療を受けていたにも拘らず，8週乃至36週後には，その網膜症の進展が認められることを経験したのである。斯くして，網膜症の進行防止乃至治療に関しては，内科治療のみに頼ることは，必ずしも十分ではなく，眼科医としても積極的にこれに参加する必要があるとの見解に達した。而してその方法が容易であり副作用の認むべきものがなく，効果を或程度期待し得るものを目標として検討した結果，先ず全身的薬物療法を試みることとした。

Autors divided the cases of diabetic retinopathy into three groups, i. e. contrast group, first and second groups. In the contrast group (14 cases), no special treatment for retino-pathy was performed. The first group (23 cases) was treated with vitamin B₁₂ (100mcg weekly), vitamin C, hesperidin and rutin. For the second group (6 cases) intramuscular injection of testosterone propionate was given 50mg weekly in addition to the treatment for the first group. All cases were constantly under strict control for diabetes. Ophthal-moscopic examinations were performed every four week extending over thirty-six weeks.
