眼科領域における副腎皮質ホルモン療法の検討

緒言

　副腎皮質ホルモン剤は1949年Henchらにより関節リウマチに対して臨床的に使用されて以来，その強力な抗炎症作用故に適応範囲は拡大し，眼科領域においても重要な位置を占める薬剤となつている。

　眼科領域における副腎皮質ホルモン剤の使用法には全身投与法と眼科特有の局所投与法があり，それぞれ劇的な効果をえている。その反面，多彩な副作用とprotective effect^1）をえる量を使用するので下垂体・副腎皮質系に対する抑制が大きな問題となる。現在われわれが日常行なつている方法について下垂体・副腎皮質系に与える影響を知ることは是非必要なことである。

Corticosteroid therapies were studied, on va-rious uveitis and optic disturbances, by the measurement of plasma cortisol and urinary 17- OHCS.

Massive intravenous administration starting with 200 mg prednisolone (Masuda's method) was effective in Harada's disease with extensive exudation and optic disturbances resulting in abrupt visual deterioration. However, this me-thod caused strong suppression and delayed re-covery of pituitary-adrenal function. We then used ACTH-Z after withdrawal of corticosteroid following tapered administration and found quick and good recovery of that function.
