早期単性緑内障の長期観察成績

Long-Term Follow-up of Early Simple Glaucoma

緒言

　古くより緑内障の視機能の予後は不良とされており，現在でも，もつとも失明率の高い眼疾患の一つである。

　単性緑内障の長期観察成績についてはすでに多数の報告があり，眼圧調整と視機能障害の関係，薬物療法と手術療法との比較，失明率などいろいろな面から検討がなされている。しかし，これを早期緑内障の長期観察成績に限つてみると，その報告は意外に少なく眼十分な検討がなされているとはいえない。これは，緑内障の初期においては視機能障害の進行が緩慢であり，その変化をとらえるには非常に長期間の観察を要することと，視機能障害の程度が軽微であることが多く，あまり関心が払われることがないことなどに起因するものと思われる。因にLeydhecker^1）は視機能障害出現前に眼圧の調節不良期間は実に20年近くにも及ぶとしているが，同一患者を10数年にもわたつて観察し続けることは非常に困難なことである。

The present study attempts to determine the fate of early simple glaucoma on the basis of long term follow-up of the disease. The crite-ria for the diagnosis of early simple glaucoma were defined by us as follows : 1) Early stage of glaucomatous field defects belonging to St-age I and II after Kozaki and Inoue²⁾ ; 2) Intr-aocular pressure level of more than 21 and sometimes raised to 30 mmHg ; 3) Impaired facility of outflow and constantly elevated P_o/ C value ; 4) Positive provocative tests in the presence of high intraocular pressure.
