妊娠を契機に球後視神経炎様症状で発症した髄膜腫の1例

A case of meningioma masquerading as retrobulbar neuritis during pregnancy

　妊娠第6週の34歳女性が左眼の眼痛と視力低下で受診した。矯正視力は右1.2，左0.15であり，左眼下方に視野欠損があった。Marcus Gunn瞳孔が左眼にあった。眼底に異常がなく，球後視神経炎などの視神経障害が疑われた。2週間後に眼痛が増強し，左眼視力が手動弁に低下した。画像診断で左蝶形骨縁内側部に腫瘍が発見された。摘出された腫瘍は微小囊胞型髄膜腫（microcystic meningioma）であった。左眼視力は術後17日目に0.8に改善し，眼痛と視野は軽快した。腫瘍はプロゲステロンレセプターが陽性，エストロゲンレセプターが陰性で，妊娠が腫瘍の増大に関与した可能性が推定された。妊娠中に球後視神経炎様の症状があるときには，髄膜腫を鑑別診断に加えるべきである。

　A 34-year-old female presented with pain and blurring in her left eye. She was during 6th week of pregnancy. Her corrected visual acuity was 1.2 right and 0.15 left. Her left eye showed Marcus Gunn pupil and inferior visual field defect. Funduscopy showed normal findings,suggesting retrobulbar neuropathy. Her left visual acuity deteriorated to hand motion 2 weeks later. Diagnostic imaging showed a tumor in the left sphenoidal ridge. Surgically removed tumor proved to be microcystic meningioma. Her visual acuity improved to 0.8 17 days after surgery. The tumor was positive for progesterone receptors and negative for estrogen receptors. There is a possibility that pregnancy may have accelerated the tumor growth. Meningioma has to be included in the differential diagnosis in the presence of pregnancy and retrobulbar lesion.
