糖尿病性網膜症の凝固・線溶系

Blood Coagulability, Blood Fibrinolytic Activity and Platelet Function in Diabetic Retinopathy

緒言

　糖尿病性網膜症（以下本症と略す）における網膜出血は，直接視力に影響を及ぼす重要な問題であり，その機序についても以前よりいろいろ論じられてきた。一般的に出血は血管壁の障害，血小板系の異常，血液凝固系の障害，循環抗凝血素の量，そして線溶系の亢進などいろいろな要因により影響を受け，一元論的には結論は下されない。

　本症における網膜出血も，従来線溶能の亢進と関連を持つとの報告もあり^1）〜3），また最近では糖尿病患者では線溶能は抑制傾向を示すことが多いという事実から，福田^4），宇山ら^5）は線溶能の低下傾向を指摘している。そしてこれらのことは，治療面にも直接影響をきたす事実でもあり，著者らは本症の出血に関与する凝固系，線溶系および血小板について諸種検索を行ない，比較検討を加えたしだいである。

The blood coagulability, blood fibrinolytic ac-tivity and platelet function were investigated in 46 cases (22 male and 24 female ones) of diabetic retinopathy. The cases were divided into the following two groups : Group A (11 cases-7 males and 4 females-in Scott's stage 0 to II) and Group B (35 cases―15 males and 20 females-in Scott's stages III to V).

The results obtained were as follows :

1) Blood coagulability-The amount of fi-brinogen was within normal limits or apt to increase in almost all cases examined and its tendency was noted especially in the male cases in Group B.
