原発性アルドステロン症（Primary Aldosteronism）の1例

Fundus Change in Primary Aldosteronism

緒言

　原発性アルドステロン症は1955年Conn^1）によりはじめて記載された疾患で，本症では周期性四肢麻痺，発作，多飲，多尿，脱力などの臨床像があり，高血圧，低K血，腎濃縮力低下が100％例外なくみられているといわれている。本邦においては1957年，鳥飼^2）によりはじめて報告されて以来，年ごとにその症例数は増加しているが，眼科領域からの報告は少ない。今回本症の1例を経験し，その術前，術後の眼底変化を観察する機会を得たので報告する。

Primary aldosteronism which produces hyper-tension, hypokalemia and aldosteronura is a"curable" hypertension. In this paper, the author reports the improvement in ophthalmoscopic fin-dings, especially the changes in the caliber of the retinal arteries before and after the surgical treatment of the disease, in a 47-year-old female. Aldosterone in urine was 21 ng/dl. Systemic blood pressure was 200/140 mmHg and diffuse narrowing of the retinal arteries was marked by ophthalmoscopy before the treatment. The general condition as well as ophthalmoscopic changes improved after subtotal adrenalectomy.
