Behçet病の免疫抑制療法—臨床症状に対する効果

Immunosuppressive Therapy of Behçet's Disease.:Its Effect on Clinical Syptoms

緒言

　Behçet病の原因は，現在なお不明であり，したがつて其の治療法も未だ確立されていない。本疾患の臨床像，経過，病理組織所見等より考えると，本症の発症にはいわゆる免疫異常による膠原病類似の病態がうかがわれ，したがつてこの方面からの治療が行なわれつつあるのが現状である。すなわち，Cyclophosphamide,6—mercaptop—urine等に代表される化学的免疫抑制剤が本疾患に試みられ，文献的にBuckleyら^1），森田^2），青木ら^3），大藤，山名^4），Mamoら^5）が有効であつたと報告している。

　われわれは今回，完全型10例，不全型6例の計16例のBehçet病患者にステロイド剤（STH），Cyclophosphamide （CP），6—mercaptopurine（6MP），Azathioprine （AP）などの抗炎症，抗免疫剤を長期間投与し，その臨床症状とくに眼症状に及ぼす影響を検討し新知見をえたので報告する。

The results of immunosuppressive therapy in 16 patients with Behçet's disease (10 cases of complete type and 6 cases of incomplete type) are reported. In the treatment, cyclop-hosphamide (CP), 6-mercaptopurine (6 MP) and azathiopurine (AP) were used in combination with steroid hormones (STH).

Patients with relapsing ocular symptoms for more than one year showed resistance to the immunosuppressive therapy, although better therapeutic effects were obtained in cases where the immunosuppressive treatment was began within 3 months after the first appear-ance of ocular symptoms.
