眼窩腫瘍に対するMicrosurgical Techniqueの応用

Application of Microsurgical Technique to the Orbital Tumor

緒言

　眼窩腫瘍は，統計上眼疾患の約0.01〜1.47％^13）を占めるに過ぎないといわれている。しかし，その治療に関しては予後はもとより，眼球を保存しうるか否か，あるいは眼球を保存しえた場合でも眼球運動障害，視力，視野障害，その他眼瞼下垂等の眼機能障害をおこさぬようにするという点で，きわめて重要な問題を含んでいる。

　われわれは，昭和44年1月1日より昭和47年12月31日までの4年間に，青森県立中央病院，脳神経外科において9例の眼窩腫瘍を経験し，主として経前頭開頭により腫瘍を摘出してきた（第1表）。さらに最近では，microsurgical techniqueを応用することによつて，眼窩内の神経，血管，筋肉等に与える損傷を最少限度におさえ，術後の眼機能障害をなるべくきたさぬように努力してきた。

In our series of 9 cases of orbital tumor, two cases were operated on under the operating microscope. The first case is a 22-year-old female and the second is a 2-year-old boy. Both patients were hospitalized with complaints of unilateral exophthalmus.

For the operative method of the orbital tumor many ophthalmological approaches have been performed sine before. On the other hand, neurosurgical approach is conducted by means of transfrontal craniotomy. When the orbital tumor is extirpated and is especially benign, it is most important to preserve the eye-func-tion at the best condition.
