局所過敏症を応用したブドウ膜炎の診断

Clinical Application of Local Hypersensitivity in Diagnosis of Uveitis

緒言

　感染にもとづく内因性のブドウ膜炎の病因診断には，病変部より直接病原菌を分離，同定することが必要である。しかし，日常の臨床では，早期に抗生物質の投与が行なわれるために，病原菌の同定は必ずしも容易なることではない。また，前房穿刺や硝子体穿刺などは，病変の軽度なときには，侵襲が大きすぎるので，routineにはおこなわれないこともある。そして，先人の報告や病理学的検索のできた症例の臨床所見を参考として，全身所見，血清学的所見より，眼病変の原因を推論しているのが現状といえる。

　今回，私たちは，局所過敏症を応用したブドウ膜炎の診断法を紹介するとともに，その理論的基礎，臨床的応用としての結膜ツベルクリン反応をもちいる眼結核症の診断法を検討した結果，本法の成績とその問題点について報告する。

A total of 95 patients with various types ofuveitis was tested as to hypersensitivity to tuberculin applied subconjunctivally.

As a preliminary measure, intracutancous tu-berculin test was conducted at various dilutions ranging from 2,000×to 100,000,000×. Thereafter, 0.1ml of tuberculin at the minimum dilution that causes negative skin reaction was injected subconjunctivally. Cases were judged as positive when conjunctival hyperemia simulating scleritis was present 48 hours following subconjunctival injection.
