緒言

　網膜剥離手術にさいして，手術操作はほぼ順調に遂行され，手術終了時には網膜の復位が期待されたにもかかわらず，術後に予想外の合併症の出現により，網膜の復位に複雑な経過をとつたり，不成功に終わつたり，または視機能の回復の不良をみる症例に時に遭遇する。これらの術後合併症の原因を探り，その予後を調査して，その反省のもとに，手術操作の改善をはかることは網膜剥離の手術成績の向上に重要なことと思われる。

　ここで，われわれの最近4年間の網膜剥離手術症例の中から，剥離手術の術後合併症のうち，網膜の復位や，術後の視機能に影響を与えた重篤な合併症の発現したものをえらんで，その種類，発生頻度，予後に及ぼす影響を調査し，それぞれにつき，原因，対策の検討を加えた。

Five hundred and twenty-five cases with retinal detachment have been treated in 604 surgical sittings in our clinic for the past four years. Various kinds of severe postoperative complications were observed and reoperation be-came necessary in 74 eyes. Excessive penet-rating diathermy frequently resulted in severe complications as massive vitreous retraction and macular pucker among others. Scleral buckling procedure was followed by infection in 3 and uveitis in 5 instances. Successful reattachment of the retina could not be attained in most cases with giant retinal breaks or massive vit-reous retraction.
