悪性網膜剥離の治験例

Treatment for severe cases of retinal detachment

　網膜剥離の眼底所見や症状は，誠に多様であり，これに対してすべての症例に万能であるという標準の手術術式はない。対象が視機能と直接に関係のある網膜であり，手術侵襲はこれを損傷することをできるだけ少ないものを選び，しかも，剥離を治癒せしめるのに十分な手術をためらうことなく初回手術時に行なわなければならない。ディアテルミー凝固法は，剥離網膜の復位手術に大きな貢献をしてきたが，その後の鞏膜切除術Scleral resection，鞏膜締結術Scleral bucklingの発展は，ディアテルミー凝固法で治癒せしめえぬ重症例をも救い，治癒成績を一段と向上せしめた。現在では，われわれは，もつばらディアテルミー凝固法のみに依存していたころと異なり，種々の術式をもつている。したがつて，各症例ごとにもつとも適した術式を選択施行することが多く，網膜剥離の復位手術は，一段と多彩なものとなつてきている。

　本稿では，われわれが最近取扱つた重症例に対する術式を，2,3の症例を付しつつ記述したい。

There is no operation which is successful in 100% of patients with detached retina. In this regard, the surgeon should perform suitable operation for each case. This is a discribtion of the authors' recent treatment for severe cases of retinal detachment. Cases where lamellar scleral resection, scleral infolding of Chamlin, equatorial suture of Arruga, scleral buckling with polyviol implant of Custodis and scleral buckling with polyethylene tube of Schepens were performed were presented. The paper does not cover the statistic result, complication and follow up study of each operative procedures.
