侏儒（Dysplasia spondyl epiphysaria congenita）と巨大裂孔を伴つた特異な網膜剥離

Two Cases of Bilateral Retinal Detachment with Giant Breaks Associated with Congenital Spondyloepiphyseal Dysplasia

緒言

　Congenital spondyloepiphyseal dysplasia（先天性脊椎骨端異形成）あるいはSpranger—Wiedemann症候群と呼ばれる，主として躯幹骨の発育不良による侏儒があり，これに網膜剥離が合併することはこれまでにもいくつかの報告例があり，最近注目をあつめている^1）3〜5）。しかし，その報告された症例の多くは網膜剥離に関しては陳旧例であり，新鮮例の報告は非常に少ない。わずかにLalive d'Epinayら^3）の報告した1例とBrégeatら^4）の報告した1例とがあるのみである。これら2症例はいずれも硝子体の変化が強く，治療は困難を極めたようであるが，巨大裂孔を生じたものではない。

　私は2例のSpranger-Wiedemann症候群の症例において，3眼に巨大裂孔を伴う網膜剥離の新鮮例を経験したので，ここに報告する。

The first patient was a boy of nine year old. On his first visit, he had a giant oral disinser-tion of 270° with rolled edge in his right eye. The refraction of both of his eyes was highly myopic. The right eye was treated by light coagulation but without success. After two we-eks the whole retina was detached and rolled up to small cluster upon the optic nerve head. After six months of this event he developed another giant oral disinsertion of 300° in his left eye following a most insignificant trauma.
