対眼窩腫瘍放射線療法による脳損傷例

A Case of Brain Damage due to Radiation Therapy for Orbital Malignancy

緒言

　眼窩腫瘍の手術療法にも限界がある場合があり，また，抗癌物質の薬剤療法にも副作用のため使用制限されることがあり，放射線療法の対眼窩腫瘍療法における比重は強い。しかしながら放射線療法にもその副作用はまぬがれないところである。

　小原らは，かつて対眼窩腫瘍放射線療法により，眼窩に最も近い髄膜に炎症および壊死を起こした症例を報告し，かかる副作用を防ぐため，原体照射療法（クリヌキ照射療法）を用いるべきであることを提唱した。しかし，私どもは，眼窩腫瘍に原体照射〔回転照射〕を行なつたが，再発および耳前リンパ腺への転移等のため4回にわたる照射を行なつたので，脳に重大な変化を生じた例を報告し，その脳の病変の機転について，従来報告されているごとき100〜500μの動脈のみならず，実に大きい血管にも変化が現われているのを知つたので，ここに報告する。

A 55-year-old female had developed orbital tumor and was operated on by removal of the tumor and orbital exenteration. The tumor proved to be basal cell carcinoma so that post-surgical radiation therapy was initiated. The dosis of radiation during one course ranged from 1050 r to 5, 048 r and the total dosis of radiation, which was applied in 5 divided courses duringthe period of 19 months, amounted to 23,098 r.

About 3 years and 3 months after initiation of radiation treatment, the patient developed aphasia, acalculia, disturbed orientation and retrograde amnesia.
