網膜剥離術後の早期離床について

Minimized Bed Rest Following Retinal Detachment Surgery

緒言

　術後10日間は両眼包帯下に絶対安静を守らせ，2週間後に歩行を開始させるというのが1969年までわれわれの採用してきた網膜剥離の術後安静度であつた。しかし次第に「これほどの苦痛を患者に強いなければ網膜剥離というものを直すことができないのだろうか」，「われわれは患者に不必要な負担を強いているのではないか」という疑問を抱くようになり，1969年後期より徐々に安静度の軽減を試み始めた。途中，われわれと意見を全く同じくする清水^1），佐藤^2）の報告を読み勇を得るとともに，自らの症例数が増すにつれ早期離床の実施について確信が持てるようになつたので，1970年5月より今回報告するような安静度の実施に踏み切つた。今回の報告は1971年9月までの分をまとめたものであり，症例数は多いとはいえないが，地方の一個人病院としての一つの成績を発表することは決して無駄なことではないと考えて，報告する次第である。

A new procedure was introduced for the care of patients following retinal detachment surge-ry. Immediately after the operation, patients were allowed to walk back to the ward with only the operated eye patched. Complete bed rest was recommended on the day of surgery. The patients were given ordinary food from the next morning on and were allowed to goto toilet. Otherwise, a relative bed rest was recommended until the second postoperative day. From the third postoperative day, the patients were allowed to leave bed and walk about in the hospital area.
