涙嚢鼻腔吻合術後の長期観察例—特に吻合部の内視鏡ならびにX線造影所見について

Long-term Followup after Dacryocystorhinostomy, with Particular Reference to Endoscopic and Roentgenologic Findings of the Site of Anastomosis

緒言

　涙嚢鼻腔吻合術Dacryocystorhinostomy （以下DCRと略す）は，閉塞した鼻涙管のバイパスとして涙嚢から中甲介前方の鼻堤に開口し，総鼻道に通ずる涙液の流出路を造設する方法である。これによつて涙液のうつ滞は全く解消されるので，流涙の停止，涙嚢内に充満していた膿の消失など見事な結果が得られる。

　しかし，DCRの古典であるToti法による成績は満足されるものではなかつたので，いく多の改良が加えられ，涙嚢と鼻腔との粘膜を縫合するDupuy-Dutemps法^8）に至つて，その優秀性が明確に立証された。

A total of 93 dacryocystorhinostomic operation in 82 patients has been conducted during the past 16 years. Satisfactory follow-up observation has been possible in 87 eyes (76 cases). Func-tional cure was attained in 75 eyes (86.2%) and improvement in 7 eyes, resulting in an over allsuccess rate of 94. 1%.

Endoscopic observation could be made in 58 instances. Large opening at the site of anasto-mosis was observed in 47%, small opening in 28%, a mere trace in 22% and atresia in 3%.
