絶対緑内障ならびに無水晶体緑内障に対する鞏膜虹彩切除術の一新法

A New Method of Scleroiridectomy for Absolute and Aphakic Glaucoma

I．緒言

　絶対緑内障や白内障手術後に起こつた無水晶体緑内障は，ときに眼圧のコントロールが困難で手術にも手こずることがあるが，前者には従来，眼球内容除去術，眼球摘出術，および各種の瘻孔手術が行なわれており，後者にはジアテルミー手術や種々の瘻孔手術が試みられている。すでに失明している絶対緑内障眼に対しても，患者の自己眼球の保存意欲は通常強いものなので，著者らは眼圧下降の強力かつ確実な鞏膜虹彩切除術の一新法を考案して絶対緑内障眼に試みたところ，おおむね満足すべき結果を得ることができ，また本法を無水晶体緑内障眼に試みてもよい結果が得られたので，ここに大要を報告する。

A new method of scleroiridectomy has been developed for cases of absolute or aphakic gla-ucoma in which conventional antiglaucomatous operations have usually failed. The procedure,which is simple in technique, free of complica-tions and effective, is conducted as follows.

The bulbar conjunctiva is incised 10mm from the limbus at the 12 o'clock position. After baring the sclera by formation of limbus-based conjunctival flap with the Tenon's capsule atta-ched to the latter, a 4mm-long scleral incision is made 5mm from and parallel to the limbus.
