We evaluated the binocular function in subjects with unilateral aphakia or pseudophakia. The aniseikonia and stereopsis were evaluated with the patient wearing contact lens or ordinary eyeglasses whenever necessary. The degree of aniseikonia was measured with the new aniseikonia test developed by Awaya. The stereopsis was measured with the Titmus Wirth test.

In the group corrected with intraocular lens (N = 57), the mean value for aniseikonia was 2.8% and the stereopsis was 5.16 on the Titmus circle. In these 57 subjects, 39 subjects showed aniseikonia of 3.0% or less which is considered to be the limit of toler-ance. The stereopsis was 100 sec, or better in 38 subjects.

In the group with aphakia corrected by ordinary glasses (N =6), the mean value for aniseikonia was 16.5%. Fine stereopsis was obtained in none.

These findings indicate that intraocular lens im-plantation is the most effective means to achieve postoperative binocular function in unilateral apha-kia.
