脈なし病網膜動脈血逆流現象—（第2報）体位の影響

Reversed Blood Flow of the Central Retinal Artery in Cases of Pulseless Disease.:II. Influence of Position of the Body

I．緒言

　第30回日本中部眼科学会の席上，私達は脈なし病患者で網膜中心動脈血圧（以下CAPと略記）を測定する際に，被検眼の眼球壁にCAPの最高値を超える圧を加えた時，網膜中心動脈内の血液が乳頭方向に逆流する事がある事を報告した。ところがその際，金沢大学米村大蔵助教授は，本現象が患者の体位によつて影響される旨の追加をされた^1）。そこで，私達は，すでに第1報で報告した患者のうちで，その後も引き続き来院中の4名について，6カ月間，来院の都度，患者の坐位と仰臥位とでそれぞれCAPを測定して，体位の如何によつて逆流現象の出現に差異があるか否かを調べた。

　その結果，1例で，しかもただ1回ではあつたが坐位では逆流現象が認められ，仰臥位では，これが認められなかつたので，改めてここに報告し，諸参の御叱声を乞う次第である。

Out of the 4 cases reported earlier, one female case with pulseless disease showed peculiar pattern in her reversed blood flow phenomenon of retinal artery elicited by ophthalmodynamo-metric procedures through bulbar compression.

In this subject, a 32-year-old female, the phenomenon was positive in her sitting position during her menstruation period. The pheno-menon was absent when supine and in any body positions during her non-menstrual periods while undergoing systemic stroid treatment. It was found that her CAP values, both systolic and diastolic, were significantly lower during menstruation than otherwise.
