経強膜水晶体摘出術の試み

An attempt of trans-scleral extraction of lens

Ⅰ．まえがき

　従来の水晶体摘出術は嚢外にせよ嚢内にせよすべて角膜輪部（即ち隅角部）を切開して虹彩を排除して水晶体を摘出する方法で，この方法では眼圧調整上重要な隅角部を切開して，更に大なり，小なり虹彩に侵襲を加える等により，術後緑内障或は虹彩脱出，又は虹彩炎等を惹起し，又多くの場合瞳孔偏位を起こし，術後視力障害の大きな原因となる事が多いのは，日常我々が経験している処である。又他方硝子体内の非磁性異物或は硝子体内嚢腫等を摘出する場合，強膜を切開して目的物に達するため，所謂開窓術なるものが開発されている今日，硝子体脱出或は出血等は大した問題ではなく，従つて強膜を切開して水晶体を摘出する本法は水晶体摘出のみから考えれば一見無な方法の様であるが，技術の改良工夫をはかれば本法は更に将来発展してその応用範囲も拡大すると思われるので，敢えてこの試みを発表して御批判を乞う次第である。冒険がなければ進歩はない，コロンブスのアメリカ発見はその当時では途方もない無謀なくわだてであつたに違いない。

The authors described a new method for lens extraction in which an approximately 13-14mm long section, parallel with the limbus of cornea, was performed in the sclera at the distance of 8-9mm from the limbus cornea. The lens was extracted through this section by the aid of a loop which was inser-ted into the vitreous behind the lens.

Two cases of cataract were operated by this method. A slight vision (0.1) was maintained for half a year after operation in one case, and for a month in othercase, but thereafter the retinal detachment occurred and the vision was lost in both cases.
