角膜周辺拡張症の1例

A case of the marginal keratectasia

緒言

　Schmidt-Rimpler^1）のいわゆる慢性周辺溝角膜炎Chronische periphere Furchenkeratitisは，比較的稀な疾患であり，現在迄のところ，ドイツ，フランスを中心に約130例の報告がある。本疾患は，角膜周辺硬化萎縮症Randsklerose u．Randatrophie d．　Hht．（Fuchs）^2），慢性角膜辺縁溝形成症chronische periphere Rinnenbildingd．　Hht．（Gilbert^3），Axenfeld^4）Handmann^5）等），角膜縁辺変性症Randdegeneration d．Hht．（Seefelder^6）），角膜周辺拡張症periphereEktasie d．　Hht．（Ischreyt^7）Fruchte^8）Rupp—recht^9），Lauber^10）等），角膜辺縁ディストロフィーdystrophia margsnalis corneae （Junius^11）Terrien^12）等）など，種々の名称で呼ばれている。本疾患の本態に関しては，未だ不明の点が多く，此等の報告の多くは，その本態の究明に関するもので，治療面の報告は数少い。我々は，最近本疾患の典型的な1例を経験し，これに手術を行いほぼ満足すべき結果を得たのでここに報告する。

A typical case of marginal keratectasia is described. The patient was a 37-year-old female. She stated that her eyes had been repeatedly inflamed for short periods during the last twenty years, and she complained of failing vision of both eyes several years ago. Uncorrected Vision was 0.1 each and skiacopy showed extreme mixed astigmatism. Both eyes showed advanced stages of corneal dys-trophy, with typical marginal ectasia about 2mm. in width in its center, extending around practically the whole upper part of the cornea.
