白内障手術後の合併症として起つた硝子体の角膜後面接着による水泡性角膜炎の1例

A case of bullous keratitis from vitreous contact following cataract extraction

I．緒言

　硝子体の角膜後面接着による角膜浮腫は白内障手術の稀ではあるが，最も重篤な合併症の一つである。一部に限局することもあるが，徐々に増悪して結局全角膜をおかし或は激しい水泡性角膜炎となり，屡々続発緑内障を合併し，完全に行われた白内障全摘出術の結果が失明に終ることがある。

　本症はかなり以前から知られていたらしいのにも拘らず，従来殆んど特別に述べた者はなく，1948年Reeseが始めて記載し，其後Leahey （1951）の詳細な報告をみるのみである。私共は最近本症の1例に遭遇したので以下簡単に報告する。

Total extraction of mature cataract was successfully performed by sliding with erys-ophake, using α-Chymotrypsin of 1 : 5000 dil-ution in saline for minute.

On examination with slit lamp, the upper half of cornea revealed marked edema with vi-treous contact on the posterior surface on the 10th day after the operation. The corneal les-ion involved only the upper half from begin-ning to end. Bullous keratitis developed 2 months after the operation and ultimately it became leucoma, the visual acuity resulting in light perception..
