Abstract. Purpose:To report a case of temporal arteritis who manifested bilateral anterior ischemic optic neuropathy(AION). Case:An 84-year-old female presented with impaired visual field in the right eye since 5 days before. She had been treated for systemic hypertension and diabetes mellitus. Findings:Corrected visual acuity was no light perception right and 1.5 left. Both eyes showed pallor of optic disc. The left eye showed visual field constriction. Pulsed corticosteroid therapy was started under the tentative diagnosis of AION. She also showed leukocytosis and elevated erythrocyte sedimentation. Biopsy of right temporal artery showed granulomatous vasculitis leading to the diagnosis of temporal arteritis. There has been no recurrence for 4 months until present with visual acuity of no light perception right and 1.2 left. Conclusion:Early treatment with systemic corticosteroid resulted in preservation of vision in one eye in a case of temporal arteritis with bilateral AION.
