Abstract. Purpose:To report a case of bilateral anterior ischemic optic neuropathy following infectious endocarditis. Case:A 73-year-old male presented with sudden loss of vision in the left eye. He had systemic hypertension,diabetes mellitus,anemia and been receiving hemodialysis for renal failure. Findings:Corrected visual acuity was 0.8 right and no light perception left. There was no difference in light reflex between right and left eye. Both eyes showed pale and edematous optic disc with blurred margin. The right eye showed inferior horizontal hemianopia. Fluorescein angiography showed prolonged arm-to-retina circulation time over 30 seconds in both eyes. He was diagnosed with bilateral anterior ischemic optic neuropathy. Systemic corticosteroid resulted in improved visual acuity of 1.2 right and 0.3 left 6 months after onset. Conclusion:There is a possibility that infectious endocarditis was pathogenetically involved in the onset of bilateral anterior ischemic optic neuropathy in this patient.
