Abstract. Purpose:To report a case of temporal arteritis with anterior isehemic ophthalmopathy and central retinal artery occlusion(CRAO)as the initial manifestations. Case:A 69-year-old female presented with failing vision in the left eye. She had had severe headache since one month before. Findings:Corrected visual acuity was 1.2 right and 0.4 left. The left eye showed swollen and hyperemic optic disc. Pulsed corticosteroid therapy was started under the diagnosis of optic neuritis. The right eye developed CRAO 2 days later with visual acuity of 0.02. The left eye showed signs of optic atrophy. Elevated ESR, CRP and advanced age led to the diagnosis of temporal arteritis. Acute symptoms of the disease subsided 13 months after her initial visit with visual acuity maintained at 0.09 right and 0.06 left. Conclusion:This case illustrates that temporal arteritis may be the underlying lesion in a patient with advanced age, severe headache and optic neuropathy.
