Abstract. Purpose:To report a case of herpes simlex keratitis with unusual manifestations in the corneal epithelium. Case:A 39-year-old male presented with pain and lacrimation in both eyes since 10 days before. His wife was suffering from labial herpes. Findings:Corrected visual acuity was 1.0 in either eye. Both eyes showed scarring in the eyelids. The cornea in both eyes showed epithelial defects with numerous branchings that simulated an arabesque pattern. Both eyes were treated by abrasion of epithelium followed by soft contact wear. Herpes simplex virus was isolated from the abrased cornea. The patient was positive for anti-HSV IgM antibody,leading to the diagnosis of herpes simplex keratitis. Stromal keratitis occurred but was brought to cure after topical acyclovir and corticosteroid. Conclusion:This case illustrates that herpes simplex keratitis may show unusual corneal lesions.
