唐草状の角膜上皮炎を呈した両眼性単純ヘルペス性角膜炎の1例

A case of bilateral herpes simplex keratitis with epithelial defects simulating arabesque pattern

要約　目的：両眼に非典型的な角膜上皮病変を呈した単純ヘルペス性角膜炎の症例の報告。症例：39歳男性が10日前からの両眼の疼痛と流涙などで受診した。同時期に妻が口唇ヘルペスに罹患していた。所見：矯正視力は左右とも1.0で，両眼の眼瞼に痂皮化した皮疹があった。両眼の角膜に唐草状に枝分かれする上皮欠損があった。病変部の角膜上皮を擦過し，治療用ソフトコンタクトレンズ装用で治療した。擦過物から単純ヘルペスウイルスが検出され，血清の抗単純ヘルペスウイルスIgM抗体が陽性で，単純ヘルペスウイルス性角膜炎と診断した。角膜実質炎が続発したが，アシクロビルとステロイドの局用で軽快した。結論：単純ヘルペスウイルス性角膜炎は，時に非典型的な病像を呈することがある。

Abstract.　Purpose：To report a case of herpes simlex keratitis with unusual manifestations in the corneal epithelium. Case：A 39-year-old male presented with pain and lacrimation in both eyes since 10 days before. His wife was suffering from labial herpes. Findings：Corrected visual acuity was 1.0 in either eye. Both eyes showed scarring in the eyelids. The cornea in both eyes showed epithelial defects with numerous branchings that simulated an arabesque pattern. Both eyes were treated by abrasion of epithelium followed by soft contact wear. Herpes simplex virus was isolated from the abrased cornea. The patient was positive for anti-HSV IgM antibody,leading to the diagnosis of herpes simplex keratitis. Stromal keratitis occurred but was brought to cure after topical acyclovir and corticosteroid. Conclusion：This case illustrates that herpes simplex keratitis may show unusual corneal lesions.
