Abstract. Purpose:To report a case of anterior ischemic optic neuropathy in both eyes following interferon therapy for chronic hepatitis C. Case:A 65-year-old man presented with impaired visual acuity in the right eye since 2 days before. He had had hepatitis C for several years and had been treated by peroral ribavirin and subcutaneous injection of peginterferon α-2b since 7 weeks before. Findings:Corrected visual acuity was 0.8 right and 1.0 left. The right eye showed marked swelling of optic nerve head with flame-shaped hemorrhage. The left eye showed moderate disc edema. Perimetry showed inferior hemianopia in the right eye and relative scotoma in the left. He was diagnosed with bilateral anterior ischemic optic neuropathy. After immediate discontinuation of ribavirin and interferon,he was treated by pulsed corticosteroid followed by tapering. Visual acuity in the right eye recovered to 1.0 with moderately improved visual field 10 weeks later. The left eye showed normal visual field. Conclusion:This case illustrates that systemic interferon may induce bilateral anterior ischemic optic neuropathy.
