Abstract. Purpose:To report a case of retinal pigment epitheliitis suspected of varicella-zoster viral infection. Case:A 60-year-old male presented with impaired visual acuity with myodesopsia in the left eye since one month before. Corrected visual acuity was 1.5 right and 0.8 left. The left eye showed numerous cells in the aqueous, disc edema, edematous opacity of 1 disc diameter across in the posterior fundus, and grayish-white patches in the midperiphery. Optical coherence tomography showed irregular thickening of retinal pigment epithelium. Fluorescein angiography showed dye staining of edematous opacity and window defect corresponding to patches in the midperiphery. Visual acuity deteriorated to 0.01 one month later. White patches coalesced and became atrophic. Inflammatory findings disappeared 2 months later. Varicella-zoster virus was isolated from the aqueous. Conclusion:The observed retinal pigment epitheliitis may have been caused by varicella-zoster virus infection.
