塩酸ラロキシフェンにより増悪したと考えられた結核性網膜血管炎の1例

Tuberculous retinal vasculitis in a case following treatment with systemic raloxifene

要約　背景：塩酸ラロキシフェンは選択的エストロゲン受容体モジュレータで，骨粗鬆症に対して経口投与される。静脈血栓症は本剤の禁忌とされている。目的：塩酸ラロキシフェンの投与開始後に発症した結核性網膜血管炎の報告。症例：65歳女性が3週間前からの左眼霧視で受診した。霧視を自覚する2週間前から塩酸ラロキシフェンを投与されていた。所見：矯正視力は左右とも1.0で，両眼に眼底出血と網膜血管炎，左眼に豚脂様角膜後面沈着物と隅角結節があった。ツベルクリン反応が強陽性であった。塩酸ラロキシフェンの投与を中止し，デキサメタゾンの点眼を開始した。3か月後に眼底出血と血管炎は軽快した。内科の診察で結核性胸膜炎が疑われ，抗結核薬による治療が行われた。初診から15か月後の現在，眼底の経過は良好である。結論：塩酸ラロキシフェンの全身投与が網膜血管炎の発症または増悪に関係した可能性がある。

Abstract.　Background：Osteoporosis is often treated by systemic raloxifene. Thrombosis is regarded as contraindicated to this treatment. Purpose：To report a case of tuberculous retinal vasculitis detected after start of systemic raloxifene treatment. Case：A 65-year-old female presented with blurring of her left eye since 3 weeks before. She had been receiving raloxifene since 5 weeks before. Findings：Corrected visual acuity was 1.0 in either eye. Both eyes showed retinal vasculitis with hemorrhage. The left eye showed keratic precipitates and nodules in the chamber angle. Tuberculin test was strongly positive. She treated by topical dexamethasone after discontinuation of raloxifene. Fundus findings improved 3 months later. She was diagnosed with tuberculous pleuritis and treated by antituberculous medications. She has been doing well for 15 months until present. Conclusion：There is a possibility that systemic raloxifene served as trigger for manifestation of retinal vasculitis.
