未治療緑内障患者に対するラタノプロスト単独投与の効果

Effect of topical latanoprost as the initial treatment for glaucoma

要約　目的：初期緑内障に対するラタノプロスト点眼の効果の報告。対象と方法：未治療の原発開放隅角または正常眼圧緑内障35例69眼を対象とした。年齢は31～76歳（平均53歳）であり，眼圧は14～28mmHg（平均19.2±2.8mmHg）であった。眼圧は点眼開始から4週，8週，12週，6か月の時点で測定し，静的量的視野は治療前と6か月後に測定した。結果：眼圧は点眼開始後すべての時点で有意に低下した。6か月後の眼圧下降率は平均20.4％で，治療前の眼圧が高いほど下降率が大きかった。10眼（14.5％）では下降率が10％未満であった。点眼前後で視野に変化はなかった。結論：初期の開放隅角緑内障に対するラタノプロスト点眼で，眼圧下降効果と視野維持効果が得られた。

Abstract.　Purpose：To report the outcome of topical instillation of latanoprost for early-stage glaucoma. Cases and Method：This study was made on 69 eyes of 35 patients with untreated primary open-angle or normal-tension glaucoma. The age ranged from 31 to 76 years,average 53 years. Intraocular pressure（IOP）ranged from 14 to 28 mmHg,average 19.2±2.8 mmHg. IOP was measured 4,8,12 weeks and 6 months after start of instillation. Quantitative static perimetry was performed before and after 6 months of treatment. Results：IOP decreased significantly at each measurement. It decreased by 20.4％ after 6 months of treatment. Decrease of IOP was greater in eyes with higher baseline IOP. Ten eyes（14.5％）showed less than 10％ of IOP decrease. There was no change in visual field before and after 6 months of treatment. Conclusion：Instillation of latanoprost induced decreased IOP with preserved visual field for 6 months in eyes with early-stage glaucoma.
