Abstract. Purpose:To report a case who received 23-gauge vitrectomy for postoperative vitreous hemorrhage. Case:An 82-year-old woman was referred for elevated intraocular pressure(IOP)in both eyes. She had been diagnosed with exfoliation syndrome and received cataract surgery with intraocular lens implantation 4 years before. She had IOP over 50 mmHg in both eyes one week before. Findings:IOP was 24 mmHg in either eye. Visual acuity was 1.2 right and 1.0 left after capsulotomy. IOP in the left eye increased to 30 mmHg 11 months later. IOP decreased to 10 mmHg after trabeculotomy and trabeculectomy. Hyphema spread to the vitreous necessitating 23-guage vitrectomy 4 weeks later. The filtering bleb was well preserved with minimum inflammation and IOP of 12 mmHg. Conclusion:23-gauge vitrectomy induced minimum damage to the conjunctiva or filtering bleb.
