Abstract. Purpose:To report a case who developed endophthalmitis after microincision vitrectomy for retinal detachment. Case:A 64-year-old female was diagnosed with rhegmatogenous retinal detachment in the left eye. The eye received phacoemulsification-aspiration,23-gauge vitrectomy,and replacement of vitreous by sulfur fluoride. The scleral wound was sutured by absorbable material. After an uneventful course the following day,hypopyon appeared the next day. The eye was treated by irrigation of anterior chamber and vitreous. Residual triamcinolone acetonide was removed from the vitreous. Enterococcus faecalis was cultured from the aqueous 4 days later. There has been no recurrence of inflammation or retinal detachment thereafter. She is doing well 8 months after surgery. Conclusion:Endophthalmitis is a possibility after microincision viteous surgery. We advocate thorough disinfection of the conjunctiva and complete dissection of herniated vitreous.
