ラタノプロストとの併用療法におけるブリンゾラミド1％点眼液による眼圧，角膜および全身への影響

Effect of topical treatment with 1％ brinzolamide and latanoprost on the cornea, intraocular pressure and systemic findings

要約　目的：ラタノプロストとブリンゾラミドの併用療法による眼圧，角膜，全身への影響の報告。対象と方法：ラタノプロストとβ遮断薬の点眼を受けている原発開放隅角緑内障24眼，落屑緑内障2眼，高眼圧症2眼の合計28例28眼を対象とした。年齢は45～84歳（平均68歳）であった。眼圧は18.6±2.7mmHgであった。β遮断薬を1％ブリンゾラミド点眼に切り替え，点状表層角膜症，中心角膜厚，角膜内皮，血圧，脈拍数を検索した。結果：眼圧は点眼液を変更してから1か月後に16.7±2.8mmHg（p＜0.05），3か月後に16.3±2.8mmHg（p＜0.01）と有意に低下した。点状表層角膜症のADスコアは26眼（93％）で不変，1例（4％）で悪化，1例（4％）で改善した。中心角膜厚，角膜内皮，血圧，脈拍数については，点眼液変更の前後で有意な変化はなかった。結論：開放隅角緑内障に対するラタノプロストとブリンゾラミドによる併用療法は安全で有効である。

Abstract.　Purpose：To report the effect of combined treatment with brinzolamide and latanoprost on the cornea, intraocular pressure and systemic findings. Cases and Method：This prospective study was made on 28 eyes of 28 patients who were receiving topical instillation of latanoprost and beta-blocker. The series comprised of primary open-angle glaucoma 24 eyes, pseudoexfoliation glaucoma 2 eyes and ocular hypertension 2 eyes. The ages ranged from 45 to 84 years, average 68 years. The intraocular pressure（IOP）averaged 18.6±2.7mmHg. Topical beta-blocker was switched by 1％ brinzolamide. Results：IOP decreased significantly to 16.7±2.8mmHg（p＜0.05）1 month after switching, and 16.3±2.8mmHg（p＜0.01）3 months after switching. AD score for superficial punctate keratopathy improved in 1 eye（4％）, remained unchanged in 26 eyes（93％）, and worsened in 1 eye（4％）. There was no change in central corneal thickness, population of corneal endothelial cells, blood pressure or pulse rate. Conclusion：Combined treatment with brinzola-mide and latanoprost is safe and effective for open-angle glaucoma.
