強膜炎に対するトリアムシノロンのテノン囊内注射後に生じた眼圧上昇にトリアムシノロン除去術が著効した1例

Removal of triamcinolone was effective for elevated intraocular pressure after subtenon injection of triamcinolone for scleritis

要約　目的：強膜炎に対してトリアムシノロンをテノン囊内に注射し，続発した眼圧上昇にトリアムシノロン除去が奏効した症例の報告。症例：34歳男性の両眼に発症した強膜炎に対し，プレドニゾロンの点眼と内服が7か月間行われた。その後トリアムシノロン（トリアムシノロンアセトニド）12mgをテノン囊内に注射した。それまで正常であった眼圧が，右37mmHg，左45mmHgに上昇したが，2か月後に正常化した。さらに2か月後に眼圧が再上昇し当科を受診した。経過：眼圧が下降しないため，受診から4か月後に左眼球結膜下にある白色のトリアムシノロン塊を除去し，3週後に眼圧は正常化した。続いて右眼に同様の処置を行い眼圧は下降した。以後現在までの2年間，眼圧下降薬を点眼しているが，眼圧は低値を保っている。結論：トリアムシノロンのテノン囊内注射後に続発した眼圧上昇に対し，トリアムシノロン塊の除去が有効であった。

Abstract.　Purpose：To report a successfully treated case of elevated intraocular pressure（IOP）after subtenon injection of triamcinolone acetonide for scleritis. Case：A 34-year-old male was treated by topical and systemic prednisolone for scleritis in both eyes. He received subtenon injection of 12mg triamcinolone in each eye 7 months later. He was referred to us after IOP started to rise to 37mmHg right and 45mmHg left. Clinical Course：Both eyes showed elevated IOP resistant to conventional treatments for 4 months. He received removal of white subconjunctival mass of triamcinolone in the left eye. IOP nomalized 3 weeks later. A similar treatment induced normalization of IOP in the right eye. Both eyes showed normal IOP values for 2 years until present with topical hypotensive drugs only. Conclusion：Removal of triamcinolone may induce normalization of elevated IOP after its subtenon injection.
