Abstract. Purpose:To report a case of carotid cavernous sinus fistula(CCF)with narrowing of ipsilateral superior ophthalmic vein. Case:A 78-year-old female presented with hyperemia of her right eye since 6 months before. She had systemic hypertension and had surgery for retinal detachment in the left eye 13 years before. Findings and Clinical Course:Her corrected visual acuity was 0.15 right and 0.05 left. The right eye showed dilated and tortuous vessels of bulbar conjunctiva, proptosis, and impending central retinal vein occlusion. The left eye showed enophthalmos. Magnetic resonance imaging showed attenuation of right superior ophthalmic vein. A thrombus was present by angiography. Cerebral angiography showed shunt formation between intradural abranch of internal carotid artery and right cavernous sinus, leading to the diagnosis of CCF. Transvenous embolisation was followed by improved manifestation and visual acuity of 1.0 five months later. Conclusion:Superior ophthalmic vein may be attenuated in CCF due to embolism or occlusion. Cerebral angiography is useful in the diagnosis.
