Abstract. Purpose:To report a fulminant case of endogenous endophthalmitis due to Bacillus cereus. Case:A 74-year-old male was hospitalized for rectum cancer. He received tube insertion for intravenous hyperalimentation(IVH). A few hours later, he noted pain and blurring of vision in his right eye. When seen the following day, his corrected visual acuity was no light perception right and 0.7 left. Under the diagnosis of panophthalmitis, he was given massive dosis of systemic and intravitreal broad-spectrum antibiotics. The infection progressed rapidly, resulting in enucleation of the affected eye on day 7 of disease. B. cereus was isolated from the vitreous. Systemic examination failed to identify the original site of infection. Conclusion:B. cereus may frequently cause exogenous endophthalmitis. It may cause endogenous endophthalmitis as opportunistic infection as in the present case.
