涙小管に発育した乳頭腫の1例

A case of papilloma of the canaliculus

要約　目的：涙小管に発症し，摘出後マイトマイシンを点眼した乳頭腫の症例の報告。症例：49歳女性が1か月前からの左眼内眼角に生じた隆起性腫瘤で受診した。疼痛や眼脂はなかった。所見：左眼の内眼角にカリフラワー状の有茎腫瘤があり，大きさは5mm×3mm×4mmであった。下涙点が拡張し，圧迫で腫瘍が圧出された。腫瘍を切除した。病理診断は扁平上皮乳頭腫（squamous papilloma）であった。10日後に再発し，摘出したのち0.02％マイトマイシンを点眼した。2か月後に再々発し，切除したのち0.04％マイトマイシン点眼を5日間続けた。以後2年間，経過は良好である。結論：涙小管に生じた乳頭腫に対し，摘出後のマイトマイシン点眼は再発防止に有効であった。

Abstract.　Purpose：To report a case of papilloma from the lacrimal canaliculus treated by excision and instillation of mitomycin. Case：A 49-year-old female presented with a tumor in the inner canthus of her left eye since one month before. She had no discharge or ocular pain. Findings：A tumor was present in the inner canthus of the left eye. It simulated a cauliflower and measured 5 mm×3 mm×4 mm. The lower punctum was dilated. A tumor mass was expressed by applying pressure. Resected tumor was diagnosed with squamous papilloma. The tumor recurred 10 days later and was resected followed by instillation of 0.02％ mitomycin. It recurred 2 months later and was resected followed by 0.04％ mitomycin. The patient is doing well for 2 years until present. Conclusion：Instillation of mitomycin was apparently effective in preventing recurrence after resection of papilloma of the canaliculus.
