Abstract. Purpose:To report a case of tumor in the optic nerve head that later proved to be metastasis from gastric carcinoma. Case:A 63-year-old male presented with acute loss of vision in the right eye. He had noticed visual field defect in the right eye 2 months before and had been diagnosed with optic neuritis. Findings:Corrected visual acuity was no light perception right and 1.2 left. The right eye showed marked swelling of optic disc. Magnetic resonance imaging(MRI)showed no abnormal findings in the orbit or brain. The disc swelling further progressed after 2 months of diagnostic treatment with systemic prednisolone. MRI showed swelling of anterior portion of optic nerve. computed tomography showed gastric tumor that proved to be poorly differentiated gastric carcinoma by biopsy. The patient was treated by systemic chemotherapy and radiation of right eye. There has been no recurrence or metastasis during 10 months of treatment. Visual acuity in the right eye has remained the same. Conclusion:This case illustrates that asymptomatic gastric carcinoma may metastatize to the optic nerve head as a rare incident.
