角膜穿孔に至ったフザリウムによる角膜真菌症に術後ボリコナゾールが有効であった1例

Voriconazole was effective after surgery for corneal perforation in a case of fusarium keratomycosis

要約　目的：術後の角膜穿孔に対してボリコナゾールが奏効したフザリウムによる角膜真菌症症例の報告。症例と経過：60歳女性が左眼の充血と眼痛で受診した。結膜充血が主な異常所見であり，結膜炎と診断した。3週間後に症状が悪化し，左眼に周辺部角膜潰瘍があった。潰瘍が急速に拡大し，角膜擦過物の培養でフザリウムが検出された。悪化から4日後に抗真菌薬であるフルコナゾールとピマリシンによる治療を開始した。22病日に角膜が穿孔し，結膜被覆術を行った。その後ボリコナゾールの点滴と点眼を行い，病変は鎮静化し，被覆部以外の角膜が透明化した。結論：ボリコナゾールはフザリウムによる角膜真菌症に対して奏効することがある。

Abstract.　Purpose：To report successful treatment with voriconazole in a case of keratomycosis caused by Fusarium. Case and Findings：A 60-year-old female presented with pain and hyperemia in her left eye. She was diagnosed with conjunctivitis and received no specific treatment. Her left eye developed marginal corneal ulcer 3 weeks later. Fusarium was cultured from corneal scrapings. She was treated with fluconazole and pimaricin. The cornea perforated 22 days later needing repair by corneal transplantation. Newly instituted systemic and topical voriconazol led to clinical cure with transparent cornea. Conclusion：Voriconazol may be effective for keratomycosis caused by Fusarium.
