Abstract. We performed a prospective study on the corneal endothelial damage following surgery for rhegmatogenous retinal detachment in 41 eyes. The surgical modality comprised vitreous surgery 16 eyes,scleral buckling with exoplant 20 eyes,and vitreous surgery with additional lens removal and intraocular lens implantation 5 eyes. The age of patients ranged from 16 to 82 years,average 43 years. The loss of corneal endothelial cells after 3 months of surgery averaged 0.5±9.3% following vitreous surgery,2.9±8.7% following scleral buckling,and 22.0±13.0% following additional lens extraction. Eyes showing cell loss of 10% or more had intraretinal removal of corneal epithelium,rupture of posterior capsule,use of iris retractor,or postoperative fibrinous exudation. The findings show that corneal endothelial loss is partly due to inflammation in the anterior chamber.
