Abstract. We performed vitreous surgery for stage 3 idiopathic macular hole in 10 eyes of 10 patients. The cases comprised 3 males and 7 females. Their age ranged from 58 to 77 years,average 67 years. Immediately after core vitrectomy,triamcinolone acetonide was sprayed into the vitreous to visualize the vitreous gel. Triamcinolone adherent to the vitreous was then observed with a surgical microscope. This procedure enabled identification of the inner walls of the vitreous pocket and the canal of Cloquet. The vitreous pocket was about 4 disc diameters and the canal of Cloquet was about 2 disc diameters in size. A thin layer of the viteous was present between the two structures. This thin layer would exert traction to the premacular vitreous cortex when shaken by eye movements. This hypothesis can partly explain the pathogenesis of macular hole and of vitreoretinal interface syndrome.
